MEMORY IMPAIRED LIVING

Finding your place

Memory impaired living is an experience that can be difficult and challenging for those who suffer from memory impairment. Memory impairment can result from conditions such as dementia, Alzheimer's disease, or other types of neurological disorders. It can affect a person's ability to remember names, familiar places, recent events, or important dates. Memory impaired living can lead to confusion and frustration when trying to complete basic tasks such as shopping or paying bills, when remembering conversations with others, or when attempting to recall past experiences. Other effects include difficulty with making decisions, becoming easily disoriented in new surroundings, and relying on the help of others. Living with memory impairment requires patience, understanding and support from family members and friends in order to manage these challenges effectively.

At Grace Manor, memory impaired living is supported by an Integrated Care Model. This model combines both a physical and social approach to providing care for those with memory impairments. The physical component includes regular assessments of the individual’s condition such as cognitive and functional status, social needs and daily activities. This helps tailor the care plan to meet the specific needs of the individual and provide them with a safe and comfortable environment to live in. The social component focuses on interacting with the person in a meaningful way, encouraging independence and stimulating their minds through engaging activities. Staff members are trained in dementia care so that they can better understand and respond to changes in behaviour and help support individuals during times of distress. Other services available at Grace Manor include meals tailored to the individual’s taste, weekly housekeeping, medication management, therapeutic recreation programs, spiritual support and family education sessions.

OUR CARE MODEL- INTEGRATED CAREGIVING DESIGN

More than just care

As a social worker, my life is dedicated to enhancing the quality of life for aging individuals within my community. I strongly believe that our primary goal should be to provide purpose and meaning to the lives of our residents. While we maintain high standards for daily care, we are also committed to creating opportunities for engagement through meaningful activities.

Our core values are simple: prioritize meaningful and purposeful living for those in our care, recognize that each resident has unique strengths to offer, and promote daily engagement to foster a sense of purpose and creativity. We strive to help our residents live each day to the fullest by prioritizing living in the moment and encouraging intergenerational connections.

We believe that it's important to empower our residents and give them a sense of control over their lives, by involving them in the daily activities and decisions that affect them. Our care team is structured in a way that puts residents in control and makes them feel like they're leading the team. We want our residents to know that their choices matter, and we're here to support them in any way we can.

Our care extends beyond just physical well-being; we also provide support for mental health. We take pride in ensuring that our community is inclusive and welcoming to individuals of all abilities. We believe in being flexible with care team scheduling and will make arrangements to accommodate the unique needs of each team member. We firmly believe that maintaining a sense of purpose throughout all phases of life is critical, and that dignity, respect, control, and self-determination should be the guiding principles of our care philosophy.
